Overview

In this guide, we will explain how to fill in the Modelo 030 Form, which is required as part of your ES VAT application. 

Completing the Modelo Form

The following sections will need to be completed: 1, 2, 6 & 7
1. Section 1 & 2 - Data of legal representative, ID card number, day of birth and city - example below:
2. Section 6 - please insert address of registered office of the company - example below:
3. Section 7 - please enter the same data below, in section 7 which is our address in Madrid i.e. of our office in Spain:
4.  Once completed, please sign Pages 2 and 4 (Section 10) by hand, and in pen then return a full scanned copy to us by email and post the original to Madrid:
